Stores Audit Verification of Immunization Health Commodities

The objectives of this audit were to: (a) undertake full stock takes in the immunization supply chain; national, zonal, state and conduct spot-check stock at the sampled LGAs and health facilities; (b) examine whether procedures/processes implemented by government for the immunization program requires strengthening to ensure effectiveness, efficiency, transparency and accountability of the immunization supply chain; (c) evaluate the store/stock management system in the various stores in the southwest zone, States and selected LGAs/ health facilities; (d) determine the stock level (inventory balances) in each of the stores at the time of visit; and (e) suggest other /better ways/methods of re-ordering, stocking, with particular reference to respective locations (zone, States and local governments).

The store audit was conducted in the south west zone (SWZ) in Nigeria. The stores audited were the SWZ store, the six SWZ State stores in Lagos, Oyo, Ogun, Ondo, Osun and Ekiti State and 18 selected LGA stores. The activities carried out in each of the stores include: (a) stock audit; (b) record keeping audit; (c) process audit; and (d) management audit.
